- downloading and printing the registration forms (see below) to complete by hand and bring to the surgery.
Opt out – for patients who do not want the GP surgery to share anonymised data, you will need to fill out a form and send this to us: Opt out of sharing your health records - NHS (www.nhs.uk)
Named Accountable GP
You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

Removing Patients From Our List
There are 2 reasons why patients may be removed from our list:
- Out of Area – Should you move to an address that is outside our practice area we will write to you to inform you that you may no longer be registered at the practice as you live outside of our area. We will provide you with information to help you find another G.P.
- Violence and Aggression – The Partners at the Woodhill Surgery re-affirm their commitment to do everything possible to protect staff, patients and visitors from unacceptable behaviour and have a no tolerance of any incident that causes hurt, alarm, damage or distress. Appropriate action will be taken by the practice which may involve removal from the practice list and/or involvement of the police.
Temporary patients
If you are not registered with the surgery you will be unable to see one of our doctors unless you have a serious emergency. In most other cases, calling 111 is a better option.
If you are not registered but need treatment at the surgery you will need to complete a temporary registration form.
You can be registered as a temporary patient for up to three months. This means you can be registered with our practice and remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with our practice.
